Breast Surgery – A top procedure for women within the UK

Breast Enlargement Surgery, sometimes referred to as ‘a boob job’ is a very common cosmetic surgery operation within the UK.

There were 9430 Breast Enlargements performed in the UK by BAAPS (British Association of Aesthetic Plastic Surgeons) surgeons last year.

There has been a 275% increase in the numbers of patients choosing this surgery since 2002. Despite the recession, there are more cosmetic surgery procedures being carried out in the UK than ever: a 5% increase on the previous year.

What will happen during my breast enhancement surgery?

Mr. Foo will carry out a consultation, meet with you in a relaxed environment and discuss the right solution for you. You will be advised on the type of implant and placement that is most suitable to your breasts and body shape.

The procedure generally lasts an hour and is performed under general anaesthetic. Most patients are discharged the following day; some may decide to go home on the same day. However, this will depend on a number of considerations, such as where you live, your support at home and how comfortable you are after your surgery.

Types Of Breast implants

What are Breast Implants? 
Breast Implants are made of an outer shell of silicone and can be filled with silicone gel or salt water (saline). A textured surface cohesive silicone gel implant is mostly used. Texturing reduces the incidence of capsular contracture, and a better result is achieved with cohesive gel implants. Most of the time desired cosmetic results are obtained by using round implants but in some circumstances an anatomical shaped implant may be recommended.

Expandable Breast Implants
In situations such as lack of breast development during puberty (breast hypoplasia or tuberous breast deformity), the breast surgery can be done by using an expandable implant, which is a silicone implant with an inner pocket that can be gradually inflated to slowly increase volume and to stretch the deficient breast envelope. This "expansion" technique can also be very successful in young women with extremely small or underdeveloped breasts.

A breast implant can either be placed over the main chest muscle (Subglandular) or behind the muscle (Submuscular). This will depend on the amount of available breast tissue and the shape of your breasts. Increasingly, a "dual - plane" technique is used by Mr. Foo, which means he places the upper half of breast implant partially below the muscle but does not cover the lower half of the to allow better enhancement of the lower part of the breast.

Ptotic (droopy) breasts
A small degree of "uplift" does occur after breast enlargement but for patients who have experienced stretching of the breast tissue because of weight gain and loss or pregnancy, additional surgery to tighten the breast envelope and reposition the nipple (Mastopexy) may be necessary.

Determining Your Bra Size
Traditionally bra sizes have been determined by measuring the circumference of the chest wall (back) and the circumference of your chest over the breast. The difference then determines the cup size. Now, in many instances, only the back is measured and cup sizes are tried on for best fit. This could vary enormously depending on the individual. It is therefore impossible to achieve a "cup size" with breast implants. Mr.Foo advises prospective patients to experiment at home using bags of rice to determine the ideal size and type of implant. At your consultation, measurements are also taken for the Allergan BioDynamic System which may be used as a further guide to sizing.

Useful tips and advice before surgery

Stop smoking 8 weeks prior to surgery

Stop aspirin and any non-steroidal pain killers e.g. Brufen & Voltarol - 4 weeks prior to surgery

If you are planning to start a family or have more children, it is advisable to wait until your family is complete before contemplating this surgery, because despite surgery, your breasts will start to enlarge & droop again with the further weight fluctuation of pregnancy & breast feeding.

Breast Surgery is a safe cosmetic procedure

This is a very simple and safe procedure, which has been performed successfully by Mr. Foo for many years.
Minor complications:

  • Bruising and swelling
  • Asymmetry
  • Alteration of nipple sensation
  • Rippling of the implant

More serious complications:
Although every care is taken to eliminate complications and most people are not affected, but as with any surgical procedure they can occur on a rare occasion and can include:

  • Infection
  • Bleeding
  • Keloid scarring
  • Capsular contracture

NB- The capsule is a natural layer of tissue, which surrounds an implant. Approximately in 5% of cases, the capsule can thicken and contract around the breast implant. If this happens it makes the implant feel firmer and in extreme cases, it can alter the shape and make the implant feel hard. Modern implants with texturing of the implant shell have significantly reduced the incidence of capsular contracture. However, the risk of this happening is higher in regular smokers.
Rippling
In women with very small breasts, some folds of the surface of the implant may be visible as rippling on the skin surface. This risk is reduced by using cohesive gel implants which contain firmer gel. This risk can be further reduced by placing the implant under the Pectoralis muscle.
Scarring
An incision (approximately 5 cm.) is made in the crease below the breast. In most cases after insertion of the implant, the scar will be lifted onto the lower part of the breast. This incision allows accurate placement of the implant to provide a natural cleavage.
Safety of silicone
Following extensive investigation over the last 10 -15 years, silicone has been established as safe. Research has concluded that there is no scientific evidence to link silicone breast implants to conditions such as auto-immune disease. This has been confirmed by the Independent Review Group on Silicone Breast Implants (UK) in 1998 and the FDA in the United States in 2005.
Mammography and breast cancer
Women with breast implants are NOT at a higher risk of developing breast cancer. Breast implants do interfere with the view of the breast tissue in a mammogram and you must inform the radiographer so that additional X-ray views may be taken to minimise this.

All sutures are dissolvable and you will be seen again in two weeks and three months after your operation. You will be seen earlier if you have any concerns.
